You must take chemistry then organic chemistry, biology, and physics at an accredited university. You will have to take general chemistry before you take organic, however you may take the rest in any order. You should speak to the pre-medical adviser at your school to ensure you enroll into the correct classes.
If you've already taken one of these, depending on how long ago, you may want to retake it.
Try your hardest to get an A in all of them - this will UNDOUBTEDLY increase your chances of getting in.
Remember to keep volunteering (weekly), shadowing doctors for experience and potential personal recommendations (monthly), and developing strong, professional, relationships with your science professors - because these people WILL have to write your recommendations.

*If your science GPA is equal to or higher than the school's average acceptance GPA ...*
Good for you! It sounds like you're in pretty good shape, now just focus on performing well on the MCAT.
You may also consider entering a science masters program or a post-baccalaureate program during the year that you're applying.
This will help get yourself prepared for the MCAT and medical school.
It will also look extremely favorable to the admissions committee by showcasing your dedication, especially if you perform well.

You may want to consider retaking those low-grade classes to raise it before taking your MCAT.
I would strongly recommend considering enrolling into a science masters program or a post-baccalaureate program.

*The next step*
I don't need to remind you, but, remember to keep volunteering (weekly), shadowing doctors for experience and potential personal recommendations (monthly). It would also be wise to seek some sort of research position.
Now, all that is left is to take your MCAT - you should KNOCK THIS OUT OF THE PARK. Try your best to exceed the schools average.
MCAT advice can be found here.
Just ignore the part about preparing for the MCAT while in school.
